Part D Potential Exclusion Warning Reports

The 2022 Part D Potential Exclusion Warning Report 2 and the 2018 Reopening Exclusion Warning Report 1 were distributed on 5/15/2023.

Please review and take the appropriate action necessary to address the Prescription Drug Event (PDE) records found in the report as outlined in previously distributed HPMS guidance on the PDE exclusion process.
