Part B services submitted for Molecular Diagnostic testing are valid for Provider Specialties 22 (pathologist) and 69 (Independent lab) and D3 (Oncology). Valid Places of Service include: 11 (Office), 19 (Off Campus — Outpatient Hospital), 20 (Urgent Care Facility), 21 (Inpatient Hospital), 22 (On Campus — Outpatient Hospital), 23 (Emergency Room — Hospital), 50 (Federally Qualified Heath Center), 72 (Rural Health Clinic) and 81 (Independent lab). Services performed by other Provider Specialties or in Places of Service not listed will be denied.
