Immune Globulin, Not Otherwise Specified

J1599 is defined as Injection, immune globulin, intravenous, non-lyophilized (e.g., liquid), not otherwise specified, 500 mg. It is used for immune globulin products that do not have a more specific code HCPCS code assigned for a specific date of service.

When billing J1599, please remember:

  • The drug name must be provided
  • The total amount of the drug being billed must be provided
    • If billing for waste, it should be clear how much was administered and how much was waste
  • The National Drug Code (NDC) number must be provided
  • Waste should be reported with the JW modifier using the appropriate QB values
  • The Quantity Billed (QB) is based on 500 mg
    • Example: If billing a 2,000 mg dose for J1599, the QB should be 4 (4 x 500 mg = 2,000 mg).
      The claim should also have 2,000 mg in the comments field or Loop SV101-7.

The drug name, amount being billed, and NDC should be included in the comments field on the claim or Loop SV101-7.

Once an immune globulin product receives a more specific code assignment, it would no longer be appropriate to bill using J1599. Please confirm the effective date of new codes when billing.
