Helpful Tip: Wound Care

When requesting home health services for the primary reason of wound care, there must be a description of the wound included in the documentation. This could be on the face-to-face encounter, the Outcome and Assessment Information Set (OASIS), the plan of care (POC), or any other documentation submitted.

If home health services are continuing, there must be a current description of the wound in order to establish medical necessity for wound care. This can be done within, but is not limited to, the recertification OASIS or the current POC.
