Medicare Enrollment Application Fee for Calendar Year (CY) 2025

The Centers for Medicare & Medicaid Services (CMS) has issued a notice via the Federal Register announcing a $730.00 CY 2025 application fee for institutional providers/suppliers (as that term is defined in 42 CFR § 424.502) that are:

a. Initially enrolling in Medicare, Medicaid, or the Children's Health Insurance Program (CHIP);

b. Revalidating their Medicare, Medicaid, or CHIP enrollment; or

c. Adding a new Medicare practice location

This fee is required with any enrollment applications submitted from January 1 through December 31, 2025 and must be paid via PECOS. No fee payments should be submitted to the NPWest.
