Please note that the 'Medicare Secondary Payer Inquiry Form' has been updated to include a fax number where additional documentation can be sent. This is the preferred method to receive your documentation. If possible, send all information to the fax number instead of via mail services. This will ensure receipt of the information and have it placed immediately into the workflow for quicker processing.
