A DEX® Z-Code is a unique identifier that clearly identifies the test being performed on a claim, allowing the payer to know exactly what service was rendered.

The following Commercial payers have begun requiring providers to submit a Z-Code for Molecular Diagnostic Test Services on claims:

• UnitedHealthcare® Commercial

Commercial payers participating with the DEX Z-Code program will direct you to register in the DEX® Registry to obtain Z-Codes when required. Providing the Z-Code on a claim, with the appropriate CPT® code, will clearly identify the test being performed and eliminate some of the administrative burden encountered surrounding billing for these services.
