Medicare Diabetes Prevention Program Suppliers: Separate Medicare Enrollment

Published 08/06/2018

For a claim to be valid under the Medicare Diabetes Prevention Program (MDPP), you must have a separate Medicare enrollment as a MDPP supplier. If you are an approved MDPP supplier and you meet the MDPP supplier requirements and standards (including preliminary or full Centers for Disease Control and Prevention recognition), you can submit claims for HCPCS G-codes (PDF) for your services. If you do not have a separate Medicare enrollment as a MDPP supplier and you submit a claim for MDPP services, your claim will be rejected.
